What does a Medicare Special Needs Plan (SNP) cover?

A Medicare SNP is a type of Medicare Advantage Plan that specifies the insured’s coverage to their selection of care for particular diseases and medical needs. Your plan’s benefits, then, are tailored exactly to you.

To qualify for a Medicare SNP plan, you must fall into one of three categories:

  • Chronic Condition SNP (C-SNP), including diabetes mellitus and chronic heart failure or cardiovascular disorders, strokes, etc.
  • Dual Eligible SNP (D-SNP), including full Medicaid, Specified Low-Income Beneficiary without other Medicaid, Qualifying Individual, etc.
  • Institutional SNP (I-SNP)
